As we age our hormone levels decline. Both men and women experience hormonal declines. For women, the decline is most noted at menopause. Men, on the other hand may have a much more insidious decline and have their own version of menopause, called andropause.

One perspective is that the decline in hormones is “normal”. Well, dying is “natural” too but we certainly want to prevent that! I’ve had several patients tell me that they have had their hormones checked and they are “normal” for their age. They also often have joint pain, low energy, and declining libidos which could also be considered “normal”. Go enjoy it, right!? When we replace testosterone we want to make sure the testosterone is being broken down by our bodies well. Testosterone can convert to estrogen as well as dihydrotestosterone (DHT). We want to keep an eye on this conversion and optimize it so you feel the most benefit. Some doctors will, for example, prescribe testosterone replacement, but we take it a step further and make sure that all the other hormones are being optimized as well. For men, testosterone replacement will often lead to too much estrogen. Elevated estrogen can increase the risk of prostate problems and can easily be reduced with a variety of treatment options.

Also, testosterone converts to DHT which can be a good or a bad thing depending. Luckily, we can increase testosterone without increasing DHT too much. DHT, as an androgen, is about 4 times as potent as testosterone and has many positive benefits when not in excess.
